Staging lung cancer: METHODS OF STAGING (7)

Lung cancer

Mediastinoscopy cannot assess posterior subcarinal, aor-topulmonary, para-aortic and/or subaortic, periesophageal and pulmonary ligament nodes. Biopsy of the aortopulmonary and para-aortic and/or subaortic (anterior mediastinal) nodes (stations 5 and 6) is indicated if there is evidence of enlarged aor-topulmonary window nodes and cervical mediastinoscopy is negative. The primary concern is whether the nodes are fixed to the pulmonary artery because the five-year survival rate after complete resection in the presence of resectable subaortic disease only approaches that of survival after resection of N1 disease . While traditionally performed by anterior me-diastinotomy, extended cervical mediastinoscopy, which provides a single staging method for left upper lobe tumours, is performed, but only at a few centres .
